This is not a required form for sampling, we are including it for your convenience. Any format that you provide which includes this information will be accepted.

Orders of 1-25 tests are $11.50 each
26-50 tests are $11.00 each
51 tests and over are $10.75 each

Samples are conditioned and tested under standard conditions

All results will be sent in electronic PDF format. You will require Adobe Acrobat Reader 5.0 or higher.
