This is not a required form for sampling, we are including it for your convenience. Any format that you provide which includes this information will be accepted.

Please prepay three (3) tests or fewer.

Orders of 1-25 tests are $9.50 each
26-50 tests are $9.00 each
51 tests and over are $8.75 each
